In-Home Services + Care Management

Beach Cities Health District (BCHD) provides services to improve the quality of life and maintain the independence of older and disabled Beach Cities residents.

A Plan To Age Successfully At Home
Care Management provides confidential in-home visits and assessment along with recommended care plans to support independent living at home from a professional social worker.

Care Management is available for older adults (ages 60+) and disabled adults (ages 18-59) living in Redondo Beach, Hermosa Beach or Manhattan Beach. Care Managment clients may also be eligible to recieve the services below. 

Conversation Companions

Older and disabled adults who have difficulty leaving their homes benefit from having a trained, friendly volunteer make weekly social visits. These visits provide stimulation, companionship and something to look forward to, when they might otherwise have little social interaction.

Errand Assistance

The Errand Volunteer program helps older adults who are homebound or can no longer drive maintain their independence. Trained volunteers can drive clients to the grocery store or pharmacy, or even do their shopping for them.

In-Home Exercise

BCHD’s physician-approved MoveWell In-Home Exercise program helps older adults become stronger, healthier and feel better through simple, safe and easy exercises. An exercise specialist visits to determine which exercises would be most beneficial; then a trained volunteer is matched and provides weekly in-home visits and phone calls to provide support.
