Care Management
Beach Cities Health District’s Care Management helps residents remain in their home as long as safely possible.
BCHD’s Care Management is available for older adults (60+) and disabled adults (18- 59) living in Redondo Beach, Hermosa Beach or Manhattan Beach. Care Management provides confidential in-home visits and assessment along with recommended care plans from a professional social worker. Links to related community services are also available.
